Evaluator Membership Renewal

PHAA member evaluators pay an annual $15 fee to remain members. Membership ends in August of the year that appears in parenthesis next to your name below. If you were a PHAA member in the past but don't appear on the listing below, you will have to snail-mail in the Evaluator Application Form (click on forms at the top of this page) with your check or money order.

As a PHAA member, you can put together transcripts for your evaluees and take advantage of other PHAA services for evaluators including a listing on our evaluators page (which can include your bio and photo) and access to an evaluator's forum where evaluators communicate with each other.

PHAA will no longer accept your membership renewal unless you check the box below, certifying that your state-mandated child abuse clearances are up to date (within the past five years). Click here for more info about this requirement and how you can comply with it.
