Use this form after you have graduated and have obtained your final PHAA transcript (the version with places for the parent and graduate to sign) from your evaluator.

If you specify that you will be attending the PHAA ceremony, we will provide you with the diploma with its case at the ceremony and you can bring the signed final transcript (by parent and graduate) with you to the ceremony.

If you won't be coming to the PHAA ceremony, we will snail mail you your diploma in its case once we have received a good copy of your signed final transcript (by parent and graduate). If you can email us (phaa.office@gmail.com) a very good pdf of that signed transcript, that will be acceptable. Alternatively, you can snail mail the paper copy to PHAA, c/o Rachel Hill, 316 Farrar LN, Saltsburg PA 15681.
