You probably shouldn't be using this page. Your evaluator should submit the Common App transcript for you. He or she can download the transcript as a pdf, replete with a cover letter and the evaluation letters from the same program that he or she uses to put together the PHAA transcript. Not only that, but when the evaluator does it, he or she can fill in recommendation information about the student based upon the evaluator's knowledge of the student, something that we cannot do. If your evaluator has any questions about this process, he or she can email (phaa.office@gmail.com).
Only use this form if your evaluator won't serve as your counselor for the Common App. Instead list Dr. Howard B. Richman (drhbr1950@gmail.com) as your counselor.
